This Addendum No. 2015-04 is issued to modify or amend the Bid Documents for SUPPLY & INSTALLATION OF FIRE ENGINE COUPLED WITH FIRE PUMP AND JOCKEY PUMP (SINGLE BID ONLY) – 2ND REBID scheduled on 27 May 2015. This shall form an integral part of the Bid Documents.

1. The above mentioned item is BIDDED BY LOT.

2. Signatories for Envelope 1 and 2 should be the same person – President or General Manager.

3. Eligible bidders are required to submit the Bid Security either in the form of:
a. Cash/Cashier’s/Manager’s Check payable to the University of the Philippines Manila/Philippine General Hospital at two percent (2%) of the total Approved Budget of the Contract (ABC). Said amount should be paid at the UPM-PGH Cash Division and a photocopy of the Official Receipt should be attached to Envelope 1.
b. A combination of Cash/Cashier’s/Manager’s Check at a minimum of P50,000.00 and the balance at 2% Bank draft/guarantee or Irrevocable Letter of Credit issued by a Universal or Commercial Bank; Provided, however, that it shall be confirmed or authenticated by a Universal or Commercial Bank, if issued by a foreign bank. The Php50,000.00 should be paid at the UPM-PGH Cash Division and a photocopy of the Official Receipt should be attached to Envelope 1.
NOTE: Amount of Bid Security (Equal to percentage of the ABC) – First Php50,000.00 and the rest 2% bank draft/guarantee or irrevocable letter of credit
c. A combination of Cash/Cashier’s/Manager’s Check at a minimum of Php50,000.00 and the balance at 5% Surety Bond callable upon demand, it shall be accompanied by a certification by the Insurance Commission that the surety or insurance company is authorized to issue such instruments. The Php50,000.00 should be paid at the UPM-PGH Cash Division and a photocopy of the Official Receipt and original copy of the Surety Bond should be attached to Envelope 1.
NOTE: Amount of Bid Security (Equal to Percentage of the ABC) – First Php50,000.00 and the rest 5% Surety Bond.
d. Bid Securing Declaration

4. Bid Bulletin, Financial bid form – which includes bid prices, bill of quantities and applicable price schedules, and any other document required in the BIDS will form part of Envelope No. 2 and will be submitted together with Envelope No. 1, on or before 9:00AM, 27 May 2015 (Wednesday) at the Office of the Internal Audit, UP Manila, 8th Floor, Central Block Building, PGH Complex, Taft Avenue, Manila.

INVITATION TO BID [RE-BID]
1. The University of the Philippines Manila (UPM) invites interested BIDDERS to submit proposals for the SPORTS AND WELLNESS CENTER PROJECT (PHASE 1).
The Approved Budget for the Contract (ABC) is THIRTY ONE MILLION SEVEN HUNDRED EIGHTEEN THOUSAND NINE HUNDRED NINETY FOUR PESOS AND EIGHTY ONE CENTAVOS (Php 31,718,994.81); Source of Fund: Incremental Tuition Fund.
2. PROJECT DESCRIPTION:
The project shall cover the construction of the Sports and Wellness Center (Phase 1). The three-storey structure with mezzanine has total floor area of 3,500 sq.m.
a. Location: University of the Philippines Manila Compound, 625 Pedro Gil St., Ermita, Manila
b. Implementing Agency: University of the Philippines
c. Contract Duration: 180 calendar days
3. Open Competitive Bidding will be conducted in accordance with the pertinent provisions of the IRR of RA 9184, specifically Annex E - “Contract Implementation Guidelines for the Procurement of Infrastructure Projects”. Schedule of bidding activities as follows:

Bid Bulletin - NIH Bldg Project [Negotiated Procurement 2014 No. 1]

Query: As for the SLC, we would like to know if a Single Largest Contract amounting to at least P490M irrespective of the nature of the contract of the General Contractor plus a track record showing expertise on similar project irrespective of the amount of the Joint Venture partner is an acceptable form of SLC. Please confirm.
Answer:
No. The prospective bidder must have completed a single largest contract (SLC) that is similar to the contract to be bid. The definition of SLC for this project is as follows:
SLC and similar projects shall mean at least a medium rise* multi-level building costing at least 50% of the ABC(P 490,000,000.00). The building shall have the following: office spaces, function rooms, and mechanical systems composed of ducted air conditioning, ventilation and elevator system.
*As per the National Building Code of the Philippines (PD 1096) , medium-rise multi-level buildings shall be from six(6) up to twelve (12) storeys in height.
Query:Regarding the statement of completed projects, we would like to know if projects within the last 10 years are still acceptable. We consider five years too short to properly present the track record of the bidders especially general contractors. We recommend to the honorable BAC that the declaration of the statement of completed projects should be within the period of 10 years before bid opening.
Answer: Yes. Projects completed from Year 2000-2013 are acceptable. However, we are updating this to include 2014/before the opening of bids.

Query:For Joint Venture of PCAB contractors, a Special PCAB license is required which will require 10 days (at minimum) to apply. If the ITB will be posted in PHILGEPS today, we only have remaining 8 days including the bid opening day to process our Special PCAB license. ITB is a requirement in applying for the said license. We would like to know if the honorable BAC will accept late submission of Special PCAB license and submission of proof of filing from PCAB during the bid opening of this negotiated procurement.
Answer: No. A Valid Philippine Contractors Accreditation Board (PCAB) License Category AAA and registration for the type and cost of the contract for this project is part of the technical eligibility requirement; hence, bids that fail to include this requirement shall be considered as “failed”. Consequently, the bidder shall be declared disqualified.

SUBJECT: DESIGN AND BUILD OF THE NATIONAL INSTITUTES OF HEALTH BUILDING

The submission of bids was moved to Friday, 12 December 2014, 8:30 A.M. at the 8/F UPM Board Room.
The opening of bids will be at 9:00 A.M. of the same day, also at the 8/F UPM Board Room.
Post qualification and resolution of award will be on Monday, 15 December 2014.
Based on the Bidding Documents, specifically on Annex 4 – CHECKLIST OF ELIGIBILITY, TECHNICAL AND FINANCIAL DOCUMENTS Page 2 # 7

“CLASS B DOCUMENTS: Valid joint venture agreement (JVA), in case the joint venture is already in existence. In the absence of a JVA, duly notarized statements from all the potential join venture partners stating that they will enter into and abide by the provisions of the JVA in the instance that the bid is successful shall be included in the bid. Failure to enter into a joint venture in the event of a contract award shall be ground for the forfeiture of the bid security. Each partner of the joint venture shall submit the legal eligibility documents. The submission of technical and financial eligibility documents by any of the joint venture partners constitutes compliance.”

PCAB license of the joint venture is required as a condition for the issuance of a Notice of Award.

1. As a result of the SBAC-SWC meeting held on November 27, 2014, a Prebid Conference instead of the scheduled dropping and opening of bids will be held on December 1, 2014, 1:30 PM at the 8/F Social Hall.
2. The Submission of Bids will be on or before 12:00 noon, 15 December 2014 at the Office of Internal Audit, 8/F PGH Compound, Taft Avenue, Manila.
3. Opening of Bids will be on 15 December 2014 at 1:30 PM, 8/F Social Hall, PGH Compound, Taft Avenue, Manila.
