The Philippine General Hospital hereby informs the public that it will be procuring the following item/s through Small Value Procurement:

Supply, Delivery and Installation:

2. 2 pcs. FRU Valve Exhalation, brand new Php 141,700.00
3. 2 pcs. Safety Valve, brand new Php 115,250.00
4. 1 pc. Cable Assy Gui to BD, brand new Php 52,500.00
5. 1 pc. PSOL Valve, brand new Php 151,300.00
6. 1 pc. Filter Outlet 806, brand new Php 4,200.00

Note: Includes inspection of equipment, test performance and functionality.

Submit your quotation duly signed by you or your duly authorized representative with the following eligibility requirements on a sealed envelope not later than 12 Feb 2020 ; 3:00 PM.

I. Certified Photocopies of:

a. 2019 Mayor’s/Business Permit or its Equivalent Document
b. Latest Annual Income Tax Return;
c. PhilGEPS Registration Certificate;
