Please submit the following : 1. Price Quotation; 2. Certificate of Exclusive Distributorship; 3. Mayor's/ Business Permit; 4. PhilGEPS Certificate/Registration Number; 5. Income/ Business Tax Return and brochures for BAC consideration (Annex “H” Appendix A, Revised IRR of RA 9184).
