The Philippine General Hospital hereby informs the public that it will be procuring the following item/s through Small Value Procurement:
Supply, Delivery, Testing and Commissioning:
1. 1 unit BIOSAFETY CABINET, Class II Ductless, brand new Php 458,080.00
Internal Work dimension: Length: at least 1000mm
Depth (width): At least 580mm
Filter: Filter efficiency of at least 0.3 micra or lower (99.999%)
Work Area opening: not lower than 190mm
Material (body): Cabinet made of steel with powder coating
work zone: Stainless steel
With built-in or detachable support stand with caster wheels
With UV lamp, Low noise level: 65 or less
Electrical outlet: 220 volts
TOTAL APPROVED BUDGET……. P 458,080.00
Note: 1. Provide foot rest and AVR
2. Warranty Period: One (1) year.
3. FREE INSTALLATION, Provide training on usage and care, Initial Calibration.

Submit your quotation duly signed by you or your duly authorized representative with the following eligibility requirements on a sealed envelope not later than 16 September 2020; 3:00 PM.

I. Certified Photocopies of:

a. 2020 Mayor’s/Business Permit or its Equivalent Document
b. Latest Annual Income Tax Return;
c. PhilGEPS Registration Certificate;

II. Original Copy of:
d. Request for Proposal Form
e. Notarized Omnibus Sworn Statement
Request for proposal & eligibility requirements must be delivered to the address below on or before the specified closing date and time.
It will be appreciated if you can submit your quotation in the Bids & Awards Committee 1 office,located at 2nd flr Purchasing Office, PGH Compound, Taft Ave.,
Manila not later than 3:00 pm on 16 Sept 2020 at which time the said quotations will be opened.

This is a Sealed Proposal thus we will not accept quotations received thru email.
