The Philippine General Hospital hereby informs the public that it will be procuring the following item/s through Small Value Procurement:
Supply, Delivery, and Installation:
1 pc. LAMINAR FLOW HOODD/CABINET, brand new Php 628,923.00
- Vertical type, 6ft normal size, sliding sash
- glass slide walls, with interior height between 2.25ft - 3 ft.
- with UV lamp, stainless steel IV bar, support stand with
caster wheels, AVR, with prefilter
- Ultra Low Particulate Air (ULPA) filter with 99.999%efficiency
- ISO class 3 work zone
- Two (2) years service warranty
TOTAL APPROVED BUDGET……. P 628,923.00

Submit your quotation duly signed by you or your duly authorized representative with the following eligibility requirements on a sealed envelope not later than 23 SEPT 2020; 4:30 PM.

I. Certified Photocopies of:2 sets CERTIFIED TRUE COPY
a. 2020 Mayor’s/Business Permit or its Equivalent Document
b. Latest Annual Income Tax Return;
c. PhilGEPS Registration Certificate;

II. Original Copy of:
d. Request for Proposal Form
e. Notarized Omnibus Sworn Statement
Request for proposal & eligibility requirements must be delivered to the address below on or before the specified closing date and time.
